Please print and fill out the following forms below if you have an appointment to receive the COVID-19 vaccine.
If you are called to come to a vaccination site to receive the Covid-19 vaccine in Geary County, please previously fill out the consent form and read through the fact sheet. This is an effort to help speed up the process and ensure social distancing. If you have any questions, consult your healthcare provider.
At this time, we are scheduling every vaccine upon administering. This ensures all citizens will be getting their booster shot at the correct time. If you do not have an appointment and show up to a vaccination clinic you will be turned away. If you are unable to print the consent form ahead of time, we will have copies of these documents at administration sites if you do not have access to a computer or printer.
Appointment Instructions
A limited number of COVID-19 vaccines are now available for Phase 2 distribution in Geary County. Some people, ages 65+ who signed up using the pre-registration system are now eligible to receive the first dose of the COVID-19 vaccine.
WHO: Only the person who received the notification is eligible to receive a vaccine during the listed appointment time due to limited supply.

WHEN: Please come between the time frame listed in the communication sent to you from the Health Department.
WHY: You were contacted because you signed up to receive the coronavirus vaccine through the Geary County Health Department website.
HOW:
- Please check-in at the location given in your detailed message from the Health Department.
- Please bring valid government-issued ID and completed paperwork (consent form).
- Inside the location, a healthcare worker will administer your shot and provide you a vaccine card.
- You will then be asked to wait approximately 15 minutes for an observation period before you leave.

What to expect AFTER your vaccine:
The vaccines may cause side effects in some people, like sore muscles, feeling tired, or mild fever. These reactions mean the vaccine is working to help teach your body how to fight COVID-19 if you are exposed. For most people, these side effects will last no longer than a day or two. Having these side effects does not mean that you have COVID-19.
